Care Coordinator

Salary: £27,000 - £35,000 DOE

Hours: Monday - Friday, 8:30am - 16:30pm

Job Type: Full‑time, Permanent

Location: In person

What We're Looking For

  • Minimum 1 year's experience in the care sector (Senior Care Assistant level)
  • Previous Care Coordinator experience
  • Level 3 Health & Social Care (required)
  • QCF in Health & Social Care & Social Care Wales registration (desirable but not essential)
  • Full UK driving licence and access to your own vehicle
  • Ability to stay organised, calm and focused under pressure
  • Strong communication and people-management skills

What You'll Be Doing

  • Scheduling and matching Care Assistants to customers
  • Ensuring seamless rota cover and smooth care delivery
  • Handling new referrals and processing updates to care plans
  • Working closely with the office team to maintain accurate records and reports
  • Supporting Care Assistants to ensure they have what they need to deliver high-quality care
  • Participating in the on-call rota (1 weekday per week and 1 weekend in 4)
