Blue Arrow Swansea is recruiting for a Mental Health and support accommodation worker for one of their clients based in Rhondda Cynon Taff and Merthyr Tydfil.
Hours: 30 per week including weekends on a rota basis
Pay Rate: £17.50ph
Contract: Temporary until March 2026
Location: Rhondda Cynon Taff and Merthyr Tydfil
Role Overview
To improve the lives of people with mental health needs and other complex life challenges by providing group support and one-to-one support to equip them with the skills, knowledge, and coping strategies, connecting them with peer groups and the local communities.
This service has been developed in partnership with Cwm Taff Morgannwg Local Health Board to provide support for adults who have a mental illness and who need support to manage their mental and health and wellbeing independently.
The aim of the staff team, therefore, will be to encourage and enable clients to reach a greater level of independence and empowerment over a reasonable period. Staff facilitate group activities daily between Monday and Friday across Rhondda Cynon Taf and Merthyr Tydfil.
Main Duties
- Regular travel through the Cwm Taff region.
- Receiving referrals from professional organisations and assessing individuals, as well as receiving and assessing self-referrals.
- Identifying and meeting the needs of individuals.
- Supporting and assisting clients in group activities and advice and guidance sessions that promote health and wellbeing, community engagement and recovery, financial budgeting, pursuing leisure and educational goals, social skills development, management of medication and appointments, and independent living skills and security.
- Processing of approved new referrals - following procedures for completing new assessments, risk assessments and creating individual person-centred plans.
- Follow procedures for conducting monthly and annual reviews - or as and when required due to changes in individuals' needs or circumstances, ensuring risk assessments and support delivery plans are kept up to date and are relevant.
- Completing the Outcomes Database and producing reports for internal/external purposes.
- Completion of personal wellbeing plans along with regular and timely reviews.
- Attendance at multiagency meetings to ensure the complex needs of the client are addressed appropriately.
- Communicate effectively and professionally with individuals receiving service, Support Workers, Careers, Referrers and other Professionals via telephone, email and face to face meetings.
- Maintaining accurate records and statistics required to monitor and evaluate the effectiveness of the service.
- Data collection and appropriate recording and presentation for monitoring and Quality Assurance reports as directed by the Practice Leader.
- Follow procedures for general record keeping and information collection, saving and archiving - adhering to confidentiality and data protection policies.
- Implementing Equal Opportunities Policy and all other policies and procedures.
- Working in accordance with our clients Recovery Program.
Skills/Experience
- NVQ Competencies in Health and Social Care Level 2 or a commitment to work towards this.
- Must have sufficient experience of providing care and support services to people with serious mental illness
- Experience of providing experience of providing care and support services within a social care setting.
- Must have ability and willingness to use computers e.g., Microsoft Word, Excel, Outlook, to email, etc.
- Must possess excellent time management skills.
- Be a car owner with full license and access to own car for work purposes
- Must have sound understanding of the social care needs of people with serious mental illness.
- Must have sufficient understanding of the social care needs of people with serious mental illness.
- Have a basic understanding of the community care approach and be able to simply describe the difficulties experienced in the community by people with a serious mental illness
