Job Summary / Main Purpose
The post holder will work as part of a team in providing safe effective person centred and compassionate care for young people who have a learning disability and display behaviours of concern. This role requires the post holder to contribute to the assessment of care needs, which will include all elements of personal care for the young people and the development of care plan, risk assessment and positive behaviour support plan. The role requires the post holder to assist in the management of everyday service delivery through the role of shift co-originator, this requires them to support the staff on duty to plan meaningful and fun activities for the young people outside of their school and family times, ensuring daily tasks are completed, and management of mediation and finances in the home.
The Post holder will ensure a high standard of Care/Support is achieved in accordance with the ethos of the home, taking into account the home is a regulated service, works under relevant legislation, policies & procedures of the BHSCT.
This post will require the post holder to work a rota that covers 24 hrs per day that is open all year round, this includes working evenings and weekends, sleep in duties and possibly night duty, as is appropriate.
KEY DUTIES / RESPONSIBILITIES
- To comply with all policies, procedures, standards and guidelines of relevant professional body, for conduct and professional practice.
- To assist management in the everyday running of the home by working as a shift co-ordinator and leading shifts, having oversighting the completion of daily tasks, planning for the young people and taking responsibility for the management of medication administration and financial processes.
- Role mode and lead by example on a daily basis to the team in how to deliver safe, compassionate and effective care to the young people.
- Observe young people's behaviour i.e. change of mood or feeling unwell and attend to same, documenting and reporting action taken.
- Support young people on outings either individually or in groups, as and when required and to support the integration of young people into the local community.
- Participate fully in the team approach to the young people's care/support by contributing and attending staff meetings, consultations, group discussions and formal supervision as required to ensure that good communication and co-operation is kept to a high standard at all times.
- Staff will be required to administer and record medication as prescribed by the GP in line with the system and policy for the administration of medication.
- Staff will be required to work a rota that covers 24 hrs per day that is open all year round, this includes working evenings and weekends, sleep in duties and possibly night duty, as is appropriate.
- Ensure all emergencies, untoward events and complaints are acted upon and processed in accordance with the Trust Policy and Procedures.
- Consult and liaise as necessary with family members, community social work staff, nurses, doctors, psychologist, or other professionals as appropriate in the interests of young people.
- Assess, implement, maintain, develop and monitor young people's care plans, PBS plans and risk assessments on an ongoing basis ensuring that these are up to date and are tailored to meet the young person's social, emotional, physical and spiritual needs and meets legislative requirements.
- Take responsibility for delegated roles such as health & safety, menu planning, activities, first aid, Fire Safety, Car up keep etc. as agreed with your line manager.
- Ensure that adequate Health & Safety measures are maintained in accordance with the Trust's policy, thus ensuring the health and safety and welfare of the young people is paramount at all times.
- Undertake the direct care of children and young people, which involves ensuring that their social, emotional, health, educational, spiritual and physical needs are met as fully as possible. Direct care will include support with personal care and other practical tasks such as cooking and serving food on occasions and assisting children with the purchase of clothing.
- Undertake responsibilities in relation to specific young people and seeks to ensure that they develop social and life skills to help them transition into adulthood.
- Contribute to the effective discharge of Statutory Functions regarding the children in the Home (including LAC Reviews etc.).
- Contribute to the work of the Trust through Staff Development Reviews, Personal Contribution Plans, Personal Development Plan and appraisal.
- Under guidance and supervision, prepare and submits reports for a variety of formal and informal settings.
- Maintain accurate and up to date records as required and in line with BHSCT policy.
- Act in accordance with all Trust policies and procedures.
- Participate in staff training programmes as appropriate.

- Comply with the Human Rights and Children's Rights legislation in relation to the requirements of this post.
Experience Qualifications Registration
| Possess a relevant University Degree (in areas of social sciences) or Professional Qualification (such as a nursing, Speech and Language, Physiotherapy, Occupational Therapy or teaching) or equivalent relevant Health and Social Care based Third Level qualification.
OR
Possess 2 A Level's at above grade C AND have 2 years' experience equivalent to 16 hours per week in a health and social care role
OR
NVQ/QCF Level 3 Children and Young People Award or equivalent AND have 2 years' experience equivalent to 16 hours per week in a in a health and social care role
OR
Possess 5 GCSE's grades A - C, including English language AND have 3 years' experience equivalent to 16 hours per week in a in a health and social care role
AND Currently hold (if previously registered must be reregistered before commencement of post the new registration timeframe does not apply) or be eligible for registration with the Northern Ireland Social Care Council within 3 months of taking up the post *if support is needed with registration please contact the manager who will support you with this process before you are in post |
Other (e.g. Driving etc.)
| Hold a full license valid in the UK and have the ability to drive Trust vehicles to support the young people. 'Where disability prohibits driving, this criteria will be waived if the applicant is able to organise suitable alternative arrangements'
Experience in the use of IT including at least one of the core packages i.e. Outlook (Email) Word, Excel, Powerpoint, PARIS |
Knowledge, Skills, Abilities | Demonstrate that you are willing and committed to be part of a rota system that requires you to work a rota that covers 24 hrs. per day that is open all year round, this includes working evenings and weekends, bank holidays sleep in duties and possibly night duty, as is appropriate.

Have knowledge of The Children's Homes Regulations (Northern Ireland) 2005 and the Minimum Standards for Children's Homes Version 3 2023 and how it applies to working in a regulated service.
